Exercise has many great benefits for a person’s health. However, some medical conditions require clearance by a physician prior to joining the Fitness Center. This form below can be taken to a physician, completed and returned with the joining packet if you met the criteria for needing physician’s approval prior to starting an exercise program.
It’s unfortunate you no longer wish to continue your membership at Fairview Fitness Center. However, we understand that sometimes cancellations occur. As always, we do not require contracts and you are free to cancel your membership at anytime without penalty! Please download the cancellation form, complete and mail to:
200 Industrial Blvd Building A Dublin, GA 31021 (must be postmarked by the 10th of the current month to avoid dues being drafted for current month) or fax to 478-353-1902.
We will process your cancellation and deactivate your membership. If you wish to reactivate your membership, please stop by the front desk at Fairview Fitness Center and we would be happy to reactivate your membership!
